When caring for a spouse, parent, or loved one with dementia, caregivers face day-to-day challenges. For so many, managing dementia feels like a never-ending cycle, with no clear answers, few breaks, and no cure in sight. And just as solutions are found, new obstacles emerge.
“Caregivers are in a constant churn of medical appointments, emotional challenges, and life changes for their loved ones, all while trying to hold their own lives together,” said Jaycee Ray, HopeWest Clinical Manager. “It’s overwhelming and exhausting. But that’s why we are here, so caregivers don’t have to do it alone.”
Starting July 1, HopeWest is launching the HopeWest Dementia Support program to offer relief, education, and solutions to caregivers who need support. This initiative is part of GUIDE (Guiding an Improved Dementia Experience), a Medicare model program focused on improving the lives of those impacted by dementia.
With support from a dementia-trained team, including our Care Navigator and a dementia specialized medical provider, we walk alongside caregivers like you, offering guidance, education, resources and more.
“HopeWest Dementia Support is not here to replace existing community services, it’s here to connect the dots,” said Jaycee. “Whether helping you find a trusted home care provider, accessing meals or transportation, or simply giving you someone to ask guidance from at 2 a.m., this program becomes your guide to all things dementia related.”
Caregivers whose loved ones qualify for this program can benefit from the following:
- Comprehensive and cognitive evaluations in partnership with the individual’s primary care provider help draw a better picture of their concerns and challenges at home
- Monthly in-home check-ins to see the patient, monitor disease progression, review care plans, and identify any concerns that may go unnoticed by family caregivers. More visits can be added as needed based on changing needs or challenges.
- 24/7 helpline with direct access to a HopeWest team member
- Support from our Care Navigator helps to guide families and answer questions
- Education and coaching to help you understand the disease and prepare for what’s ahead
- Plans that offer peace of mind. This includes planning for “what ifs”
- Respite care options so caregivers can rest knowing their loved one is safe
