It focuses on providing patients with relief from the symptoms and stress of a serious illness. The goal is to improve quality of life for both the patient and the family. Transitions Program

Care in our Transitions Program is provided by a specially-trained team of doctors, nurses and other specialists who work together with a patient’s other doctors to provide an extra layer of support. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

Transitions treats people suffering from serious and chronic illnesses such as cancer, cardiac diseases such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), kidney failure, Alzheimer’s, Parkinson’s, Amyotrophic Lateral Sclerosis (ALS) and many more.

Care focuses on symptoms such as pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, difficulty sleeping and depression. It also helps you gain the strength to carry on with daily life. It improves your ability to tolerate medical treatments. And it helps you have more control over your care by improving your understanding of your choices for treatment.

We work hard to give you peace of mind. We offer various types of services and support for you and your family and it is covered by some insurance plans.

Transitions Program Services & Support

You can choose from a broad range of options, including:
  • Monthly nursing assessment and consultation
  • Access to volunteer services
  • Monthly social work consultation
  • Availability of 24-hour nursing consultation
  • Assessment phone calls by a team member according to patient’s care goals

Living With Cancer

Receiving a cancer diagnosis can be frightening and overwhelming. HopeWest Living with Cancer is here to help you and your family from the time of diagnosis throughout the duration of your illness. Our goal is to control symptoms and improve quality of life while providing an extra layer of support to help with emotional and spiritual care and in working through legal and financial concerns.

The Living with Cancer program works closely with your entire oncology team, locally and with cancer centers and specialist across the country. We also coordinate care with your primary physician and other specialists or health care providers. Each person’s journey is unique and services are tailored to meet your individual needs.

Living with Cancer Services & Support

Monthly care and support includes:
  • Visit by RN
  • Availability of 24-hour phone consultation or visit by RN
  • Visits by a social worker, chaplain or volunteer as needed or desired
  • Consultation and care coordination with your oncology team

Journeys Program

Our Journeys program is designed with your needs in mind, Journeys provides care for patients whose condition has stabilized and who no longer need hospice level of care but would benefit from ongoing supportive services to help manage their serious illness.

The program offers an individualized plan of care to help address the specific needs of each patient and family. Journeys is available for patients residing in an assisted living residence, nursing homes or those who are living in their own home. The care team comes to the patient for visits, limiting additional trips to the doctor’s office.

  • Palliative care consult visits by a physician or nurse practitioner
  • Monthly visits with a nurse, chaplain and/or social worker
  • 24-hour phone support which includes sending a care team member when needed
  • A volunteer for companionship, errands, transportation, light housekeeping, etc.

Services of this program are only available through qualified insurance plans and includes monthly visits with a nurse and if needed a social worker and/or chaplain. Journeys provides 24-hour phone support including after-hours visits from the care team if needed. Volunteer assistance is also available free of charge.
