Palliative care is specialized medical care for people with serious illnesses. It focuses on providing patients with relief from the symptoms and stress of a serious illness.

Transitions Program

We work hard to give you peace of mind. Our palliative care program—Transitions—offers various types of services and support for you and your family. It is covered by some insurance plans and is offered on a sliding fee scale.

Journeys Program

Our Journeys program is designed with your needs in mind, Journeys provides care for patients whose condition has stabilized and who no longer needs hospice level of care but would benefit from ongoing supportive services to help manage their serious illness.

Living with Cancer Program

Receiving a cancer diagnosis can be frightening and overwhelming. HopeWest Living with Cancer is here to help you and your family from the time of diagnosis throughout the duration of your illness. Our goal is to control symptoms and improve quality of life, while providing an extra layer of support to help with emotional and spiritual care and in working through legal and financial concerns.
