Palliative Care Consultations

Your primary care provider or specialist may request a Palliative Care Consultation visit from one of our experienced Nurse Practitioners or physicians. The consultation includes a visit by one of HopeWest’s experienced Nurse Practitioners or doctors to provide expert advice on managing symptoms from serious or life-limiting illnesses. They will also discuss how to best align your medical decisions with the goals you have for your health and well-being. Our palliative care Nurse Practitioner or physician will coordinate with your regular healthcare provider for ongoing symptom and stress management.

What are the benefits of palliative care?

Quality of Life
HopeWest provides Palliative Care as an option for people with serious health conditions who are still receiving curative treatment. Our palliative team focuses on relieving the symptoms and stress of progressive illness so that patients and family members can enjoy a higher quality of life.

Fewer Hospitalizations
Those experiencing frequent ER visits and hospitalizations due to frailty, pain, and complications of their illness can benefit from Palliative Care. Palliative Care can be provided in the patient’s or family member’s home or assisted living facility.

A Continuum of Care
Unlike many agencies, HopeWest is licensed to provide both palliative care and hospice services. If a palliative care patient chooses to elect hospice in the future, it ensures a smooth transition with no need to find another agency. However, palliative care does not have to be a bridge to hospice.

Where can I receive palliative care?

Palliative care is available in a variety of care settings including at home, at an assisted living or nursing home, in the hospital, or the HopeWest PACE Day Center.

Transitions Program

Care in our Transitions Program is provided by a specially-trained team of doctors, nurses, and other specialists who work together with a patient’s other doctors to provide an extra layer of support. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

Our HopeWest Transitions program provides symptom management for chronic illnesses such as cancer, cardiac diseases such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), kidney failure, Alzheimer’s, Parkinson’s, Amyotrophic Lateral Sclerosis (ALS), and many more.

Care focuses on symptoms such as pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, difficulty sleeping, and depression.

It also helps you:
•  Gain the strength to carry on with daily life
•  Improves your ability to tolerate medical treatments
• Have more control over your care by improving your understanding of your choices for treatment.

Your care team at HopeWest is driven to provide you peace of mind. We offer a variety of care and support for both you and your family.

Monthly care and support includes:
• Nursing assessment and consultation
• A volunteer for companionship, errands, transportation, light housekeeping, etc.
• Social work consultation, as needed
• Availability of 24-hour nursing consultation

Living with Cancer

Receiving a cancer diagnosis can be frightening and overwhelming. HopeWest Living with Cancer is here to help you and your family from the time of diagnosis throughout the duration of your illness. Our goal is to control symptoms and improve quality of life while providing an extra layer of support to help with emotional and spiritual care and in working through legal and financial concerns.

The Living with Cancer program works closely with your entire oncology team, locally and with cancer centers and specialist across the country. We also coordinate care with your primary physician and other specialists or health care providers. Each person’s journey is unique and services are tailored to meet your individual needs.

Monthly care and support includes:
• Visit by a nurse
• Availability of 24-hour phone consultation or visit by RN
• Visits by a social worker, as needed
• A volunteer for companionship, errands, transportation, light housekeeping, etc.
• Consultation and care coordination with your oncology team

Journeys Program

Our Journeys program is designed with your needs in mind. Journeys provides care for patients whose condition has stabilized and who no longer need hospice level of care but would benefit from ongoing supportive services to help manage their serious illness.

The program offers an individualized plan of care to help address the specific needs of each patient and family. Journeys is available for patients residing in an assisted living residence, nursing homes, or those who are living in their own home. The care team comes to the patient for visits, limiting additional trips to the doctor’s office.

Monthly care and support includes:
• Visit by a nurse
• Social work consultation, as needed
• 24-hour phone support
A volunteer for companionship, errands, transportation, light housekeeping, etc.
