The work of HopeWest PACE has filled a gap in care in Mesa County. It provides a person-centered approach to balance the medical, emotional, physical, and social needs of seniors in our program.

Our team works hard not only to support Participants, but to support their families as well. Jaime is the daughter of one of our PACE participants. Her testimonial shows the time we spend and the work we do profoundly changes lives.

With PACE, a team of health care professionals works with each individual to make sure needed services are coordinated. PACE programs get to know their Participants and design services to meet each person’s unique needs.
