HopeWest, a leading provider of exceptional healthcare and support services, is proud to announce the second anniversary of its innovative Program of All-Inclusive Care for the Elderly (PACE). Since its inception, HopeWest PACE has been dedicated to enhancing the quality of life for seniors in our community.

The PACE program, which launched in September 2021 has quickly become a cornerstone in senior care, providing comprehensive, personalized care to eligible seniors who wish to remain in their homes while receiving the highest level of support. Over the past two years, HopeWest PACE has served more than 170 participants, delivering a range of services including medical, social, and emotional support.

“Reaching this milestone is a testament to our community and the dedication and commitment of our team, as well as the trust placed in us by the aging adults and families we serve,” said Jill Orrock, HopeWest PACE Executive Director. “Our goal has always been to help older adults to live their best lives, and we are proud of the impact this program has had in the Grand Valley community.”

The HopeWest PACE program combines a team of experienced healthcare professionals, including doctors, nurses, therapists, and social workers, to create a personalized care plan for each participant. This approach ensures that each person receives the right level of care, with an emphasis on preserving their independence and dignity.

In addition to medical care, the program offers a wide range of services including transportation, meal planning, social activities, and mental health support. This all-inclusive approach not only addresses physical health but also focuses on emotional well-being and social connections, which are vital pieces of healthy aging.
