Colorado Gives Day benefits the HopeWest Dementia Support program!
The Dementia Support program at HopeWest is a comprehensive and coordinated approach to dementia care, centered on the unique needs of people living with dementia and their caregivers.
Our dedicated dementia care team consists of a care navigator, a nurse practitioner, and nurses. These professionals step in to provide support and education, medication management and counseling, as well as connect families to vital services, support groups, respite care, and transportation. Working in close collaboration with the individual’s primary care physician, HopeWest Dementia Support provides compassionate, person-centered dementia care throughout their journey. Generous support from the community ensures:
- 24/7 access to support service
- Care coordination and transitional care management
- Caregiver education and support
- Care planning
- Comprehensive assessment and home visit
- Medication management and reconciliation
- Ongoing monitoring and support of the program
- Referral and coordination of social services and support
