The work of HopeWest PACE has filled a gap in care in Mesa County. It provides a person-centered approach to balance the medical, emotional, physical, and social needs of seniors in our program.<\/p>\n
Our team works hard not only to support Participants, but to support their families as well. Jaime is the daughter of one of our PACE participants. Her testimonial shows the time we spend and the work we do profoundly changes lives.<\/p>\n
\u201cMy mom is, once again, full of life, finding happiness in the company of new friends. She is thriving and finding relief under the consistent care of physicians and specialists. And, she is content, knowing she is able to safely stay in her own home with her beloved dog,\u201d Jaime said.<\/p>\n
With PACE, a team of health care professionals works with each individual to make sure needed services are coordinated. PACE programs get to know their Participants and design services to meet each person\u2019s unique needs.<\/p>\n

From the very beginning, each individual spent time getting to know us and our circumstances, and my mom returns from the PACE Center feeling special and carefully tended to.\u201d<\/p>\n
