At HopeWest, our work isn\u2019t just about care \u2014 it\u2019s about presence. That simple idea has shaped how we walk beside people through aging, illness, and grief for more than thirty years.<\/span>\u00a0<\/span><\/p>\n When you or someone you love needs support, we take the time to understand what matters most to you. We call it \u201cmeeting you exactly where you are.\u201d And while it may sound simple, it means families trust that HopeWest will show up, listen, and walk alongside them every step of the way.<\/span>\u00a0<\/span><\/p>\n Kevin Watt, HopeWest Chaplain, knows his role is more than offering spiritual guidance and grief support. He listens without rushing. He provides comfort without condition. He helps families feel supported and safe during the most vulnerable moments of their lives. Kevin often asks grieving families: \u201cWhat does support look like for you? Is it prayer? Music and laughter? Holding hands?\u201d Whatever the answer, we meet you there.<\/span>\u00a0<\/span><\/p>\n That same approach is the thread that runs through every part of HopeWest. From grief and bereavement support to PACE, dementia, palliative, and hospice care, we\u2019ve built specialized teams of physicians, nurses, CNAs, social workers, chaplains, therapists, activities coordinators and volunteers that all work together to care for the whole person and their family.<\/span>\u00a0<\/span><\/p>\n This kind of care is a cornerstone in our PACE program, which supports older adults who want to remain living safely at home. PACE helps our participants define what matters most to them and then builds care around those goals.<\/span>\u00a0<\/span><\/p>\n For example, one participant hadn\u2019t been out of his wheelchair for three years and wished to take just a few steps again. So, the PACE team created a personal plan: first one step, then five, then fifteen. <\/span>This achievement, while celebrated by the whole team, was most importantly, exactly what this participant wanted.\u00a0 With the motivation to continue growing, he has since set additional goals with the support of his PACE team which will improve his quality of life without compromising safety.<\/span>\u00a0<\/span><\/p>\n We recently launched our Dementia Support Program, another example of meeting people where they are. We understand that dementia affects more than memory and the person with a diagnosis. Dementia reshapes daily life for entire families. <\/span><\/p>\n This program offers personalized guidance, education, and resources tailored to each family\u2019s unique journey. Whether someone is newly diagnosed or in the depths of caregiving, we help them find peace of mind, confidence, and support right when and where they need it.<\/span>\u00a0<\/span><\/p>\n Our palliative care program is another way we provide wrap-around care. Palliative care is for those living with serious illness but not necessarily nearing the end of life. Unlike traditional medicine, which often focuses on treating or curing illnesses, palliative care focuses on improving quality of life through symptom management, emotional support and coordination with other providers. <\/span><\/p>\n Whether someone is navigating complex decisions or simply trying to manage pain or fatigue, our team helps ensure that care aligns with their values and priorities, not just their medical needs.<\/span>\u00a0<\/span><\/p>\n In our hospice program, patients often tell us what they want most is to be comfortable and surrounded by those they love. Pain management, emotional support, and time well spent take priority. <\/span>That\u2019s not about giving up. It\u2019s about choosing to live the way that feels most meaningful.<\/span>\u00a0<\/span><\/p>\n Our philosophy of meeting people where they are has made HopeWest a trusted part of our communities.
