HopeWest appointed Dr. Heidi Marlin, MD, as the new Medical Director of the Program of All-Inclusive Care for the Elderly (PACE) serving Mesa County. PACE is a comprehensive health care program designed to enhance the quality of people\u2019s lives. As one ages, it sometimes becomes difficult to remain in our own home and live independently. PACE is designed to make that easier by providing medical and support services so one can be safe living at home for as long as possible. With PACE, a team of health care professionals works with each individual to make sure needed services are coordinated.<\/p>\n

“I\u2019m so excited that Dr. Marlin felt inspired to take on this role, ensuring that people are able to age, safely at home,\u201d said Cassie Mitchell, HopeWest President and CEO. \u201cWith her focus on delivery of care, she will not only ensure quality services for our PACE participants, but also provide support and direction to each of our interdisciplinary team members.\u201d<\/p>\n

Dr. Marlin, who has been with HopeWest since 2011, brings extensive experience in geriatric, palliative, and hospice medicine and expertise in chronic disease management and team-based care. Her background in Hospice and Palliative care and Family Medicine equips her with skills to manage care across various settings and support the needs of PACE participants.<\/p>\n

In her new role, Dr. Marlin is dedicated to high-standard medical care while addressing many social determinants of health. She aims to enhance the experience of PACE participants and support the teams caring for them. Through these efforts, she will contribute to a program that positively impacts health care outcomes in our community.<\/p>\n
