What services does HopeWest offer?

We offer a variety of services to support you and your family during a serious illness, long before hospice care may be needed. Call us to discuss your individual needs and how we may be able to support you with symptom management and home-based services.

Do I need a doctor's referral to get started with hospice?

No, anyone can request hospice services by calling HopeWest directly. A referral from your doctor is not needed.

Know you need help, but not ready for hospice?

Our palliative care program is here to close the gap. Palliative care is specialized medical care for people with serious illnesses. It focuses on providing patients with relief from the symptoms and stress of a serious illness. The goal is to improve quality of life for both the patient and the family.

My loved one was not under HopeWest care, can I still participate in grief support services?

Yes, our grief support services are open to anyone in the community dealing with the loss of a loved one. HopeWest offers an array of support groups where you can learn how to cope with grief and connect with others who share a common bond.

What is PACE (Program of All-Inclusive Care for the Elderly)?

As one ages, it sometimes becomes difficult to remain in our own home and live independently. PACE is designed to make that easier by providing medical and support services so one can be safe living at home for as long as possible. With PACE, a team of health care professionals works with each individual to make sure needed services are coordinated. PACE programs get to know their participants and design services to meet each person’s unique needs. LEARN MORE >>
